When you are discharged from hospital, if you need to continue to take medication, the hospital should provide this. In addition, the hospital should issue you with a list of medication needs (discharge documents) when you leave. Please arrange for this to be handed into the surgery well before your supply of medication runs out.
Hospital request for change of medication will be changed by a GP first, and if necessary, your doctor will issue you with a prescription. The Doctors will review your medication regularly, which may involve changes to your tablets, in accordance with current policies.
